Your patient's care team
Our integrated care team (ICT) includes professionals from multiple health disciplines that can help your patients stay on track with treatment plans for chronic conditions, develop healthier lifestyle habits, and manage their medications and doctor's appointments.
Our members, their primary care team and nurse care manager are at the center of our care team. The nurse care manager, a licensed registered nurse, is the main point of contact for the team and directs care as needed for a holistic experience. Improving Overall Health
The services our care team provides can help your patients make healthy lifestyle changes, like quitting smoking or losing weight. They can also help address social determinants of health and manage health conditions, including:
- Asthma

- Chronic obstructive pulmonary disease (COPD)

- Diabetes
- Heart Disease
Specific help is also available for women experiencing a high-risk pregnancy.

Connect with our care team
Primary care providers can use our Quality Care Rewards application in Availity® to refer patients enrolled in BlueCare Tennessee and CoverKids for extra support or case management services.
Your patients can connect with our care team by telephone at 1-888-416-3025 or participate in a Digital Case Management Program through our CareTN app, which offers customized support.
